(I) PATIENT PRIVACY NOTICE
AbbVie Pty Ltd (ABN 48 156 384 262) (hereinafter “AbbVie”) may process your personal data in the context of the Man Plan Program (Program) for Australian residents that have been prescribed and dispensed Lucrin (leuprorelin acetate) for palliative treatment of metastatic or locally advanced prostate cancer and in accordance with the dosing schedule registered with the Therapeutic Goods Administration (Medication).
The Program offers a range of health, wellness and logistics services for patients, and are administered by AbbVie’s patient support service providers. AbbVie may process the following categories of personal data about you, which we received from you directly, or from your health care professionals, and related third parties:
- contact data such as address, phone number and other contact details;
- personal details such as name, date of birth and gender;
- data collected or created regarding your use of our Program and services; and
- sensitive data such as health data relating to your disease or condition, medical history, treatment, reason for discontinuation, data on active management of the disease, your prescription collection status and Pharmaceutical Benefits Scheme (PBS) treatment eligibility assessment criteria.
(collectively referred to as Personal Data).
AbbVie may process such data for the following purposes:
- AbbVie use and disclose your Personal Data as part of your Program participation to administer and manage the Program, including customization of the Program to you, delivery of products and services, reminding you to take your product as prescribed or to obtain a repeat prescription of your product, and to provide you with Program support, communications and materials relating to your treatment and the Program. We may also contact your health care professional for safety information and to provide health care professionals with information about your medicine and your participation in the Program.
AbbVie may use and disclose your Personal Data in our Programs for safety monitoring, reporting and auditing, and responding to enquiries or issues in relation to our products, as well as to comply with applicable laws and regulations.
AbbVie will also use Personal Data that does not directly identify you for the compatible purposes of analytics, research and related publications, including to evaluate, develop and improve AbbVie's programs and related services, products and medicines.
AbbVie may share your Personal Data with service providers, our parent company AbbVie Inc., as a controller, or its affiliates for the purposes described above.
If you do not provide Personal Data when requested, we will not be able to manage your participation in the Program and, as a result, you may not be able to participate. (II) HEALTHCARE PROFESSIONAL PRIVACY NOTICE
Privacy confirmation to enable enrollment in the Program: I confirm that I have discussed the services with the patient and/or their carer, and they have consented to receive the services. I understand and have explained to my patient that in order to participate in the Program, their Personal Data, including health data, will be passed securely to AbbVie’s patient support service providers, who will provide additional information on the services and complete the patient’s enrollment.
AbbVie may process the following categories of Personal Data about you, as a patient’s healthcare professional, which we received from you directly or may source such information from AbbVie’s use of third-party databases:
- contact data such as address, phone number and other contact details;
- personal details such as name and gender; and
- data collected or created regarding your patient’s use of our programs.
AbbVie may process such data for the following purposes:
AbbVie use and disclose your Personal Data as part of the your patient’s Program participation to administer and manage the Program, including to contact you in relation to the Program, including patient-related updates, feedback on the Program, safety information and otherwise as necessary to facilitate your patient’s participation in the Program.
AbbVie will also use Personal Data that does not directly identify you for the compatible purposes of analytics, research and related publications, including to evaluate, develop and improve AbbVie's programs and related services, products and medicines.
AbbVie may share your Personal Data with service providers, our parent company AbbVie Inc., as a controller, or its affiliates for the purposes described above.
You will be offered the opportunity to nominate delegates (e.g. clinic nurses, clinic administrators or other authorised personnel) to assist with a patient’s participation in the Program. Delegates registered against you are linked to your master account. You agree to provide any reasonable assistance requested from AbbVie or its service providers to obtain any consents necessary to contact your delegates.
The Program’s digital platform gives you and your delegate/s access to the following services:
- a digital pathway that allows you and your delegate/s to refer patients for enrolment in the Program;
- a patient progress dashboard (Dashboard), which sets out the status of all your patients. The Dashboard is provided to assist you with the methods that they have adopted for managing your patients and should not be relied on solely for managing patients; and
- additional information, including but not limited to, Product Information, PBS Information, Minimum Product Information, Consumer Medical Information.
